• Use the back of a non-winning $10 Midwest Millions instant ticket
• Legibly enter one name, complete address and telephone number in the area indicated on the back of the ticket
• Place one or more entries into an envelope
• Affix proper postage and mail to:

• Entries may be deposited at certain events if the Lottery provides a designated receptacle for this drawing
• All Kansas mailed entries for the drawings shall be sent to the address above

Drawing Deadline:

• Kansas entries must be received no later than the morning mail pick-up in Topeka, KS on January 8, 2010 or deposited in a lottery designated receptacle no later than 5 p.m. that same day to be eligible for a preliminary drawing
• Kansas and Iowa entries selected in their respective preliminary drawings will be eligible for the January 15, 2010 grand prize drawing to be conducted by the Kansas Lottery in Topeka, KS
• To be eligible for the second round of preliminary drawings and subsequent second grand prize drawing, Kansas entries must be received no later than the morning mail pick-up in Topeka, KS on April 9, 2010 or deposited in a lottery designated receptacle no later than 5 p.m. that same day
• Those entries selected in both the Kansas and Iowa preliminary drawings will be eligible for the April 16, 2010 grand prize drawing conducted by the Iowa Lottery Authority, Des Moines, IA

Drawing:

January 15, 2010 and April 16, 2010

Grand Prize:

• One $500,000 cash prize will be awarded in each grand prize drawing
• Five $10,000 cash prizes will be awarded in each drawing

General Rules:

• The prize package is subject to the “Official Rules” for this drawing
• One drawing will be conducted on January 15, 2010 in Kansas and a second drawing will be conducted on April 16, 2010 in Iowa
• Players may enter this drawing as often as they wish, but you may only win one prize in each drawing
• A claim form will be sent to the winner of the Midwest Millions second chance drawing
• All winners shall claim their prize within 30 days from the date of the corresponding drawing or the next eligible entry shall be declared the winner
• Entering this ticket into the drawing constitutes authorization to identify publicly the name of the person whose entry is drawn
• Must be 18 years of age or older to enter the drawing
• Odds of winning depend upon the number of entries received
• Each entry is only valid for one drawing
